Each intern is to have specific paperwork completed by their site supervisor for their internship experience. Below are the specific forms that need to be completed, and the student needs to turn them into his/her University supervisor by the due date.

- The following link is a list of supervision requirements created by the NCBLPC for interns and post-graduates wishing to receive their licensure. NCBLPC indicates areas in which the supervisor needs to evaluate the student, as well as methods of supervision that are acceptable in a pre-licensed counselor gaining licensure.
