Appendix 6: DANCE DEPARTMENT INCOMPLETE FORM FOR TECHNIQUE CLASSES (revised 9/06)

The original instructor should complete Part I and have the student sign Part II (reverse side).  The instructor should give one copy to the student and place a copy in the student's file.  The student should take his/her copy to the instructor of the course in which classes are being made up. That instructor completes Part III and then gives the form to the original instructor (if still at UNCG); if not, the form goes to the Department Head, who will turn in a grade change form to remove the Incomplete.

(If the student is making up 5 or fewer classes, the score from the original semester shall determine the final grade.  If the student is making up more than 5 classes, the score from the make-up instructor shall be averaged in with the original to determine the final grade.  The final grade might be lowered if the student does not make up all absences.
The student must make up ____ (how many) classes, or the equivalent amount of time in longer/shorter classes, in order to meet the 80% participation requirement.  In order to avoid a reduction in the grade for absences, the student must make up a total of ____ (how many) classes, or the equivalent amount of time in longer/shorter classes.  The classes must be at the ____ level or above.

I have read the information on the reverse side and understand the implications if I do not make up the missed classes within six months after the original semester of enrollment.  I understand that it is my responsibility to contact the instructor of the course in which I will make up the missed classes, as early in the semester as reasonably possible, and present this form.  I understand that I should NOT register for the course in which I am making up classes.  I understand that I am expected to attend class with the same instructor regularly, and not just drop in occasionally.  I will remind that instructor to complete and return this form when I have made up all missed classes.  I understand that it is my responsibility to check with the original instructor (if still at UNCG) or the Department Head if my grade has not been changed within two weeks of the time I complete all classes.
