The Assisted Living Resident Services Coordinator will participate on an Interdisciplinary Care Team whose role is to promote the well-being of the residents in Assisted Living at Carol Woods and to ensure that they are provided high quality health care. Primary duties include: Prepare for transitions to Assisted Living. Create and oversee delivery of residents' individualized plan of care, Collaborate with multidisciplinary team to ensure high quality care and services., Participate in continuous quality improvement initiatives, Serve as a primary contact person for families of Assisted Living residents, Work with other disciplines on providing continuing education to staff, Documents on an on-going, comprehensive, and timely basis. NC Department of Health & Human Services Permanent Supportive Housing Data Specialist (Posted here January 25, 2012)
A Permanent Supportive Housing Data Specialist will be responsible for identifying data needed to document state efforts at moving persons with disabilities into permanent supportive housing. The position will identify data elements and compare with existing data bases at DHHS. The position will also work with sister-State agencies to identify data needed from them to successfully track progress on supportive housing goals and initiatives. The position will work with MMIS/DIRM to explore current and potential data needs within and exterior to state government, to include the appropriate role of Homeless Management Information Systems. The position will assist in supporting local communities in needed data collection. Activities will include writing detailed reports, monitoring program outcomes, helping with data analysis, and assisting appropriate personnel within the Housing and Homelessness Unit and others in the effective use of the information. NC Department of Health & Human Services Supportive Housing Specialist (Posted here January 25, 2012)
A Supportive Housing Specialist will help provide leadership to DHHS initiatives related to permanent supportive housing. The position will work with staff from DHHS and other State agencies involved in development of affordable housing to advise DHHS on ways to encourage increased availability and accessibility of permanent supportive housing options for DHHS consumers. The position will educate local communities on best practices. The position will collaborate with partners at the state, local and federal level. The position will have a significant role in implementation of activities related to moving persons from adult care homes, persons experiencing homelessness, and other persons being released from public facilities, into permanent supportive housing. Specific activities will include, but are not limited to: facilitate meetings of diverse populations, develop and present training materials, develop cost estimates, monitor projects, facilitate strategic planning processes, write reports, and help apply for and manage grants. The respite program provides a break to family members who are caring for an older relative who has physical or cognitive impairments. The clients come to the Centenary DAYbreak Respite Program, located in Centenary UMC, from 10:00 a.m. to 3:00 p.m. on Mondays, Tuesdays, Thursdays and Fridays. During these sessions, the seniors participate in enjoyable and meaningful activities and benefit from exercise, nutrition, socialization and appropriate stimulation. SUPERVISION: The program intern will report to the Director of Respite Care. In her absence the program intern will report to the part-time Program Assistant. QUALIFICATIONS: Bachelor's degree, with continuing studies in areas of pastoral care, counseling, geriatrics, or family therapy. Concern for and interest in older adults, ability to work cooperatively with staff and volunteer team members, and good oral and written communication skills are required. TIME REQUIREMENTS: All or portion of DAYbreak program each Monday, Tuesday, Thursday or Friday, as determined by schedule. Family Caregiver Support Specialist The Piedmont Triad Regional Council's Area Agency on Aging is seeking an energetic and detail-oriented professional for the position of Family Caregiver Support Specialist. This position is responsible for the planning, fiscal, and programmatic functions of the Family Caregiver Support Program (FCSP) for the 12 county Piedmont Triad region. The specialist must maintain up-to-date knowledge of caregiving issues and provide assistance and support to local service providers. Work tasks include advocacy, planning, outreach, training, monitoring, and program implementation. Interactions occur with the following: state, county, and local public officials, service providers, community agencies, older adults and their families. Excellent verbal and written communication skills are essential. Strong computer skills are needed. In-region travel is necessary. Short term Research Position (Posted here January 25, 2012)
Dr. Bill Tullar of the UNCG School of Bryan and Economics offers a short-term research position for a Gerontology graduate student to study the effectiveness of a project of his involving "scams" and older adults. The research project would include development of measurement tools, surveys, interviews etc. and would be managed in/near Greensboro, NC. Some limited funding is available and the research opportunity is invaluable for a Gerontology graduate student interested in research.
