  • Nominee's Curriculum Vitae or Resume (required)

  • Additional supporting documents such as newspaper/newsletter articles (optional)

  • Letter(s) of Support for this Nomination (optional)

  • Letter may be from anyone who can strongly support the nomination such as
       employers, clients, other alumni, etc.
