"The OSHA Trainer must be available for questions while you take the training. She will be available at 336-402-9065 the following dates & times. This is a clinical cell phone number. If she is on the phone with a student or another faculty discussing a question please leave a message with your question & your call will be returned as quickly as possible.
Times the trainer is available for questions:
- Monday August 18, 3 PM-9 PM
- Tuesday August 19, 10 AM-3 PM
- Wednesday August 20, 3 PM-9 PM
- Friday August 29, 10 AM-2 PM
- Wed September 10, 10 AM-3 PM
- Wed September 17, 10 AM-3 PM
- Wed September 24, 3 PM-9 PM Last time available
DEADLINE FOR COMPLETING ALL COMPONENTS FOR OSHA is October 1, 2008 (this includes signing the roster, picking up materials and completing the evaluation).

- Print the Question & Answer sheet
- Answer the questions...do not submit your answers to the Trainer. These are for
your own benefit.
- Refer back to the training slide presentation to check your answers.

Pick up required packet of information
Sign the roster that you have completed the training...you will need to have your
employee number to add to the roster
- Complete the evaluation found inside the packet. Return evaluation to the envelope
labeled OSHA Evaluation in the mail room or to the labeled folder outside office 319.
