Optical scanning is an efficient and reliable method of scoring tests and collecting data. In order for us to provide the best possible service, it will be necessary for you to devote a little time at the beginning of each test or data collection period to procedural matter. Doing so will provide for faster and more accurate service.
In the last name and first name fields enter the last and first names respectively at the beginning of the field. It is not necessary to darken a bubble for blank spaces at the end of any field. The name field is required.
The identification number field is NOT required. An instructor may choose to use this field as another form of identification in the reports. If one chooses to use this field any identication number will work. Do NOT instruct students to use their Social Security Number.
Be sure to use a #2 pencil to record your answers. Ink will not be read.
Write the Honor Code statement as instructed below.
Be sure all erasures are complete.
You may deliver and pickup tests in the TLC office, 134 McIver, on Monday - Friday, between the hours of 8 AM and 5 PM.
Purchase your scan sheets in packs from the TLC or by the sheet from the Bookstore. The scanner program will not recognize all sheet formats.
Use a blank answer sheet and carefully fill in the answers for each test or subtest. Put your name, department and phone number on the top of the form in ink. You do not need to code this information. If you are using subtests, be sure the items on each key are not duplicated and that each key is labeled.
There can be only one correct answer per item. If you want to ignore a question, leave that item blank on the answer key. If you want to allow any answer for a question fill in all bubbles for that item.
If a portion of the exam is essay or short answer, please have students write the Honor Code statement on those sheets rather than the scan sheet. If the scan sheet is the only form you will be collecting from the students, the Honor Code statement should be the final question of the exam. By not coding a choice for this item on the answer key, this question will not be included in the grade computation.
Please make sure all tests are turned and facing the same direction.
Check to be sure the TLC has the necessary software to scan the forms you use for your data collection.
All the fields on the scan sheet can be used to supply information.
The TLC does not supply any statistical analysis beyond a raw count of the number of responses for each possible response, but the data can be uploaded if additional analysis is required.
You should plan on a one to two week turn around time depending upon the time of year.
